Template information for TAC/WorkSafe patients on upfront billing

Private practices may bill patients in a variety of ways, for example, through bulk billing to Medicare, direct billing to other organisations such as WorkSafe Victoria or the Transport Accident Commission (TAC), or by upfront billing to be paid by patients on the day of treatment.

It is always good practice for your patients to provide informed financial consent prior to the provision of medical treatment. Best practice is for your patient to provide written acknowledgement and consent to the way in which the treatment you provide would be billed.

The following draft templates are provided by AMA Victoria for your use. They should only service as a starting point – please do amend these further, to suit the individual needs of your practice.

Please call AMA Victoria on (03) 9280 8722 if you require any assistance with these templates.
